Reminders for Submitting Account Request Forms

- Due to the Stay at Home Order, account request forms must be submitted via fax or email to navigators@mnsure.org. Submitting these requests via mail, may cause a delay in processing.
- Some assisters are facing the challenge of submitting the form and consumer verification documents together electronically. For this reason, assisters are permitted to submit these items separately.
- If sending via email, Account Request forms and verification documents MUST be sent via /assister-central/shared-resources/joint-policies/secure-email.jspsecure email.
